If you have been vaccinated in the Philippines and are planning on travelling to other countries that require proof of vaccination, you can apply for the Yellow Card or the International Certificate of Vaccination or Prophylaxis (ICV).
Here is a comprehensive guide on acquiring your yellow card or ICV from the Philippine Bureau of Quarantine (BOQ).

Philippine Bureau of Quarantine (BOQ)
The Bureau of Quarantine (BOQ) is the health authority responsible for formulating and enforcing quarantine laws and regulations. It is a line bureau of the Department of Health (DOH) in the Philippines. The BOQ conducts medical examinations on foreign travellers and overseas Filipinos for immigration purposes and ensures security against spreading infectious and emerging diseases and public health emergencies of international concern.
Bureau of Quarantine yellow card
The Yellow Card, or the International Certificate of Vaccination (ICV), is for travellers who got vaccinated in the Philippines and plan on flying out to international destinations requiring proof of vaccination. The Yellow Card is issued by the BOQ and is also recognized by the World Health Organization (WHO).
How to apply for a yellow card online?
The ICV or yellow card application process is simple and can be done online. Before you create your account and set up an appointment to generate your yellow card, make sure you have a valid ID card, a vaccine certificate and a passport.

Here is a step-by-step guide to applying for your BOQ vaccine certificate online:
- Create your account with the BOQ Medical Services site at https://icv.boq.ph and complete the sign-up form.
- Once you have signed up, go back to the home page and log in to your account.
- Click the ‘Services’ tab
- Then select ‘Certification Services’, and you will get the options for the available BOQ certifications.
- Click on ‘Certification of Vaccine for COVID-19’ and hit NEXT.
- You can then select how you would like to receive your yellow card (pick-up or delivery). To collect your ICV in person, you must set up an appointment with the BOQ. Check below for more details on how to book the appointment.
- Enter your personal details such as your address, purpose of travel and occupation.
- Upload the required documents, including your ID, vaccine card and a copy of your passport. Ensure that the size of the files you upload is below 2 MB.
- Select your method of payment (cash or an online payment method).
How to book a BOQ yellow card appointment?
Several BOQ branches in the Philippines are issuing the ICV or yellow card, including NCR, Luzon, Visayas, and Mindanao. If you wish to collect your yellow card (ICV) in person, you will need to set up an appointment with the BOQ at your preferred date.
You can book an appointment online by following the steps below:
- Log in to your account.
- Click the ‘Services’ tab on the top of the page.
- Then select ‘Certification Services’, and you will get the options for the available BOQ certifications.
- Click on ‘Certification of Vaccine for COVID-19’ and hit NEXT.
- Click on the calendar icon located on the right side of the page and select the earliest available date for an appointment.
- You can then choose an open slot and your preferred appointment time.
eArrival Card
Note: Earlier the Philippine government had announced that One Health Pass no longer applies in lieu of the eArrival card. However, as of now, the authorities have now ruled out the requirement to fill out theeArrival cardbefore boarding a flight to the Philippines as a “scan-and-go” system will instead be implemented. The government would set up special lanes in the country’s gateways for those who want to comply with the eArrival card.
All incoming travellers to the Philippines are no longer mandated to use the One Health Pass registration system or theBureau of Quarantine e-Health Declaration Card (e-HDC), but now they must use the recently launched eArrival Card.
E-health declaration card
Travellers (including children) must register to the Bureau of Quarantine e-Health Declaration Card (e-HDC) to declare their personal information, health status, and travel history. The e-HDC must be filled out and submitted electronically by all arriving travellers to the Philippines. Upon successful submission, travellers will be issued an OHP personal QR code allowing them to board their flight. Upon arrival in the Philippines, travellers must present this QR code for verification.
You will need your One Health Pass (OHP) personal QR code on arrival in the Philippines:
- during BOQ e-HDC verification
- at the respective help desks of OSS during the verification and assignment of your hotel
- with the PCG for the final verification of your arrival
- when you check in and check out of the quarantine hotel
BOQ quarantine certificate
If you are subject to quarantine in the Philippines, you will need a ‘Unified BOQ Quarantine Certificate’ to end your isolation period. You can download your BOQ quarantine certificate through the One Health Pass (OHP) website by entering your QR code reference and last name.
